Open Safari's page menu
Tap the button, at the left of the address bar at the bottom of the screen.

Turn on SingleForm
Tap Manage Extensions, switch on SingleForm, then tap Done.

Allow it on every website
Open the menu again, tap SingleForm, then Always Allow… and Always Allow on Every Website.
